WholeStory Health PLLC (“WholeStory Health,” “we,” or “us”) is a Minnesota professional limited liability company providing virtual whole-person care through Kate Otremba, DNP. This policy explains how we handle information collected through wholestoryhealth.com.
2. What this policy covers
This policy covers our website and our patient interest list. It does not cover your medical records. Once you establish care, your health information is protected under a separate Notice of Privacy Practices, which you’ll receive during intake.
3. Information we collect
We collect only what we need:

Interest list: your name, email address, and phone number, collected through a Microsoft Forms page.

Messages you send us: anything you include when you email us.

Website usage: basic technical information collected automatically by our website platform, such as your browser type, device, the pages you visit, and an approximate location based on your IP address.
Please don’t send health information through the interest form or by email. Neither is a secure channel.

To contact you about scheduling and updates on WholeStory Health.

We do not use your information for advertising, and we do not sell it.
5. How we share it
We don’t sell or rent your information. We share it only:

With service providers that run our website and forms, currently Wix (website hosting) and Microsoft 365 (forms and email), only as needed to provide those services.

6. Cookies and Tracking
Our website platform uses cookies, small files stored in your browser, to keep the site working and to measure how it’s used. You can block or delete cookies in your browser settings. The site will still work, though some features may not.

Stop updates: reply to any email from us, or contact us, and we’ll remove you from the interest list.

Access or delete: contact us to ask what interest-list information we hold about you, or to have it deleted.

Correct: let us know if your contact information changes.
We’ll respond to requests within 5 business days.
8. Health information
This website is not a place to share medical information, and nothing on it is medical advice. Once you establish care, your health records are kept in a secure patient portal and handled according to federal law and the Minnesota Health Records Act.
9. How we protect information
We use reputable, business-grade services and limit access to the people who need it. No method of storing or sending information online is perfectly secure, so we ask that you never send health details by email or through the interest form.
10. Children
WholeStory Health serves adults. Our website is not directed to anyone under 18, and we don’t knowingly collect their information.
11. Changes to Policy
If we update this policy, we’ll post the new version here with a new effective date.
